On this page, current clients will find valuable information, from copies of the forms and handouts used during our training to links to useful articles and websites on fitness and nutrition.

  
Forms and Paperwork 

In this section, you will find copies of the standard forms and paperwork utilized during our trainer-client relationship.  If you are a prospective client, feel free to print these forms and fill them out at your leisure before our initial consultation.  Of course, if you would rather wait until we meet, that is perfectly fine.
 
PAR-Q:  Physical Activity Readiness Questionairre.  This document guides you through the decision whether or not to begin a training program without physician approval.
 
Liability Waiver:  The legal waiver I utilize for my training clients.  Please read this through carefully and ask me if you have any questions about it.
 
General Information and Health History Form:  This form helps me to learn some important biographical information about new clients and identify common risk factors and contraindications to exercise.
 
Client Contract:  The contract we'll use to define our trainer-client business relationship.  Read it through so you'll understand my business policies.  My payment structure may be a little different from what you are accustomed to seeing.
 
 
Original Fitness Writings

In this section, you will find original articles, guidelines, handouts, and tips that I have prepared exclusively for the education of my personal training clients.  You may request future writing on a topic of interest to you by clicking here and sending me an e-mail.
 
Nutrition Guidelines:  A useful, one-page quick reference for dietary planning that includes a brief list of suggested foods.
 
Happy Holiday Fitness Tips:  Some things to remember to help avoid putting on those holiday pounds.
